The required documentation depends on the nature of your specific disability or disabilities. Please read the following sections to determine what documentation you will need in order to register.

Chronic Health, Mental Health, Sensory Disabilities and Physical Disabilities

  • The Certificate of Disability completed by a health care practitioner who is familiar with your disability; your disability falls within their scope of practice and who is licensed to make a diagnosis
  • Additional documentation related to your disability can also be provided (e.g., a report from an audiologist)
  • If you live with a mental health disability, you are not required to disclose your specific diagnosis. Your physician must confirm a mental health diagnosis exists.

Attention Deficit Hyperactivity Disorder (ADHD) and Autism Spectrum Disorders (ASD)

  • The Certificate of Disability completed by a health care practitioner who is familiar with your disability; your disability falls within their scope of practice and who is licensed to make a diagnosis
  • If you have a copy of a recent psycho-educational or neuro-psychological assessment (completed at 18 years old or older or within five [5] years prior to the date of registration), please include this in your registration package. 
  • If you have a copy of your most recent Individual Education Plan (IEP), a psycho-educational or a neuro-psychological assessment from previous schooling, please include this in your registration package.

Learning Disabilities (LD)

  • If you have a copy of a recent psycho-educational or neuro-psychological assessment (completed at 18 years old or older or within five [5] years prior to the date of registration), please include this in your registration package. 
  • If you have a copy of your most recent Individual Education Plan (IEP), a psycho-educational or a neuro-psychological assessment from previous schooling, please include this in your registration package. 

Should you require a new or updated psycho-educational assessment, we can make a referral to a private psychologist. Please do not let the cost of an assessment deter you from registering with Accessibility Services; we can discuss all available funding options when you meet with an Accessibility Advisor. 

 

Additional Information for documentation:

Concussion or Brain Injury

  • The Certificate of Disability completed by a health care practitioner who is familiar with your disability; your disability falls within their scope of practice and who is licensed to make a diagnosis

OR

  • Recent medical documentation from an appropriate health care practitioner (e.g., an Emergency Room physician, family doctor, and/or neurologist) that outlines the functional impacts of your disability and your accommodation needs
  • Additional documentation may be requested to verify the need for continued services if your condition persists beyond six [6] months

Temporary Physical Disabilities

  • The Certificate of Disability completed by a health care practitioner familiar with your disability who is licensed to make a diagnosis

OR

  • Recent medical documentation from an appropriate health care practitioner (e.g., an Emergency Room physician, family doctor, and/or neurologist) that outlines the functional impacts of your disability and your accommodation needs
  • Additional documentation may be requested to verify the need for continued services after the estimated duration of the condition
