A health check-up, more formally known as Periodic Health Visit (PHV), replaces the annual physical examination.
Longer than a typical medical appointment, a PHV offers you an opportunity to connect with the nursing team and a doctor to identify your health care goals together. You’ll be able to take a holistic approach to your health and talk to clinicians about what matters most to you and what you would like to address.
It includes a review of your health history, lifestyle and risk factors, vaccines, and a limited physical exam. While you’ll be able to discuss many aspects of your health, it’s not intended to address a single or particular health concern.
The periodic health visit is insured by OHIP once per patient per 12-month period. If a physician determines a complete history and physical examination is necessary (for a patient with chronic illness and/or health issues, for example) the physician will request a full physical exam, which OHIP will cover (Source: Ontario Ministry of Health).

Service Eligibility
- Full-time and part-time University of Toronto undergraduate and graduate students who have paid the appropriate incidental fees.
- Toronto School of Theology undergraduate students at Emmanuel College in the conjoint full-time basic degree program.
- Students in the School of Continuing Studies English Language Program (identified by a student card).

Service Fees
Periodic Health Visits are generally covered by your provincial health card or UHIP (for international and exchange students).
You will be responsible to pay for certain services not covered by OHIP such as third-party requests, for example, placement physicals or certificate of health for employment.
A third-party request for full physical or general examination is CDN $100*.
